CLIENT AGREEMENT

I have enrolled in the Monthly Mind-Body Rehab Program ("Program) offered through The Mind-Full Clinic Nutritional Consulting.

I understand that The Mind-Full Clinic is served by Jiordana Saade, CNP (“Jiordana”). Jiordana is a Certified Nutritional Practitioner and Registered Nutritionist who does not dispense medical advice nor prescribe medical treatment. Rather, she provides education through The Mind-Full Clinic to enhance my knowledge of health as it relates to diet, lifestyle, and dietary supplements.
I recognize that the the recommendations Jiordana provides are not a substitute for the diagnosis, treatment or care of disease and health concerns by a medical provider or psychologist., and that none of the recommendations she makes are meant to replace the advice of a medical doctor or psychologist. .

Nutritional or lifestyle evaluations or tests provided by The Mind-Full Clinic is not intended for the diagnosis of disease. Rather, such evaluations are intended as a guide to developing an appropriate health-supportive program for me, and to monitor my progress in achieving my personal goals. Any information I receive as a result of any such evaluations will not release the advice of a qualified health care practitioner.
I confirm that I have been advised by The Mind-Full Clinic to discuss any such information and/or recommendations with a medical doctor or licensed health care provider. I acknowledge that I am responsible for my own actions.

I agree that it is my sole responsibility to contact my medical doctor to obtain approval for participation in the Program if I am enrolling for health reasons.

I hereby agree to disclose all medical conditions to The Mind-Full Clinic. I agree to promptly inform The Mind-Full Clinic of any changes in my medical condition and treatments.

I am aware that the Program includes:

- 1 (50min) Intake Session with Jiordana ($500 value)
- 30min follow up sessions for each new step ($500/ session value)
- Voxer/ walkie talkie support ($2500 value/ month)
- Personalized Protocol ($500 Value)
- Recipes suggestions
* Supplements not included

Should I need to reschedule any Consultation, I hereby agree to notify The Mind-Full Clinic via email (at [email protected]) at least forty-eight (48) hours prior to the start of the scheduled Consultation. I understand that should I fail to provide the such prior notice, I will not be entitled to either (i) reschedule the particular Consultation, nor (ii) any type of refund or compensation. Similarly, I understand that “no show” appointments will not be rescheduled or refunded. 

I acknowledge that my enrolment and subsequent participation in the Program is purely voluntary and is in no way mandated by The Mind-Full Clinic.

By signing this contract, I confirm that I have been made aware of all payment requirements of the Program and hereby agree to comply with all such terms. I understand that the total Program fee is $2000.00 USD (the “Total Fee”), or $333 USD each month for six months. The Total Fee shall be paid in full prior to the last Consultation. Alternatively, I acknowledge that The Mind-Full Clinic may (at its sole and absolute discretion) permit me to pay the Total Fee in instalments throughout the duration of the Program, provided, that the the Total Fee is paid in full prior to, or on the one hundred twentieth (120th) day of the Program. Terms of any such instalment plan shall be agreed to in writing with The Mind-Full Clinic prior to the first Consultation (the “Payment Plan”). I agree to be bound by the terms of any such Payment Plan I may enter into in the future.

I understand that any payments I make to The Mind-Full Clinic are non-refundable.
I agree to hold harmless and indemnify Jiordana Saade, CNP, The Mind-Full Clinic, and its officers, directors, agents, representatives, affiliates and successors from any liability whatsoever resulting from my participation in the Program. I accept the risk of any decision, action or outcome based on my choice to enrol in the Program. I acknowledge that expectations and results from the Program vary among individuals and that each individual may not receive the same benefit.

The Mind-Full Clinic shall not release your personal information to others, unless it is necessary to obtain advice from other health care practitioners in order to better assist you.

I HEREBY AFFIRM THAT I HAVE READ AND FULLY UNDERSTAND THE ABOVE STATEMENTS.
